If one of my HEX exchange users, withdraws and transfer his coins to an address registered on the platform, do I still need Vault or an external wallet system?
Would This transaction get recorded on the blockchain?
Is there any transaction fee involved?

AFAIK the transactions within HEX exchange itself are not recorded on the blockchain so there should not be any fees. The internal withdrawals would be an internal accounting and is done internally.
As for the vault, the transactions are all recorded so depends on your use case I suppose.
